Young Art: Online Registration Form

Please submit this form to accompany the artwork submitted by your school to Young Art 2013: The Story of Me.
This form must be completed in its entirety by the classroom instructor or art specialist who represents the student artwork.

For each artwork submission, please enter the student name, title of artwork, and grade.
Please fill out an additional form if you are submitting more than three works.

Instructors will be notified by mail if their students’ artworks are selected or if they are not selected to be exhibited. In addition,
some submissions that will not be on view at the Museum may be placed in local libraries and community centers in Young Art displays.
